It didn´t OTL so I doubt it would do it in WW
Since Afghanistan is a popular subject at the moment how about this? Skoda T-15 of the Royal Afghan Army mid-40´s

Looks fairly reasonable. I think reliability would be the biggest factor in the Ghan with unavailability of spares. The Vickers Light Tank Mk IV might be another reasonable choice.

The barrel on the bottom design looks abit large to me.

It seems ok to me. It's a big tank, mounting a big gun. It doesn't really look out of proportion.
I thought it looked big at first but then I looked at the IS-2 and changed my opinion. The turret on the IS-2 is smaller however, more likely due to its shape.
